住院病案，也叫住院病历，是出国就医时需要提前提供的资料，病案包括：病案首页，记录了进入医院时的患者基本信息，从入院开始，到做各种检查，包含影像报告，病理化验报告，到手术记录单，到药单，医嘱，出院证明都需要翻译成英文，提供给国外医院医生参考，是否收治由国外医院决定。
病案需要有医学背景的专业翻译员人工翻译，翻译完毕后需要加盖翻译宣誓书，包含翻译员资格证书编号，名称，宣誓词，翻译机构地址电话翻译员签字等信息。
病案文件翻译注意事项：1.病理名词，2.一并名称，3.检查报告中的医学用语。
我们接触过大量的病症，一般都是国内医院治不了的疑难杂症，主要是癌症，骨科，血液病，抑郁症，神经内科，神经外科，心脑血管等。当然也有综合病例，比如：妇科、消化，内分泌，呼吸科，肾病，脑病。因为子女在国外的原因，也顺便去美国看，那就带着在国内医院拍的片子，国内医院医生出具的病历，包含：诊断报告，曾经治疗过的过程，手术方案，医嘱，药单等病理文件和就诊卡等，提前翻译好这些病案，预约后，到美国治疗.

3.The contact details of the working unit; (翻译人员所在工作单位地址及联系方式的详细信息；)
4.The translator's signature; (翻译人员签字；)
5.The date of the translation; (翻译日期；)
